IMMUNIZATION POLICY

The District of Columbia Immunization Law requires that all students who are under the age of 26 by July 1st and are Fall Semester entrants at the University must provide proof of certain immunization prior to registration. 

 

Newly-admitted undergraduate, graduate, non-degree, or transfer students for the Fall Semester under the age of 26 by July 1st are required to provide proof of immunization or they will incur a registration hold on their student account.

Immunization forms are due by June 15th for Fall Semester entrants.

New Students

New students under the age of 26 will be able to register for their first semester without a health form on file. However, a hold will be placed on your account to prevent future registration if all required vaccination records are not submitted to the Student Health Center. All immunization forms are submitted online through the student portal.

These records will take our staff some time to review and we appreciate your patience as we review your records carefully. Please check your portal regularly for updates regarding your immunization requirements. For questions, please email [email protected]. Please provide your GWID in your message.

PLEASE NOTE: Your immunization records will be reviewed in the order received. Thank you for your patience while we carefully review your records. If the team has any questions or concerns about your submission, they will reach out to you directly through the student portal. Please check your portal regularly for updates regarding your immunization requirements.

Fall 2024

Fall Semester entrants who are under the age of 26 by July 1st will need to submit the Mandatory Health Form (PDF) through their student portal. Prior to submission, the Mandatory Health Form will need to be completed and signed by the student’s Primary Care Provider. Please retain a copy of the completed form for your records. It is the student’s responsibility to ensure that their healthcare provider completes and submits the form to the Student Health Center by the June 15th deadline.

How to Submit Your Immunization Record

In order to properly submit your immunization record, please follow these instructions:

  1. Access your student portal and sign-in with your GW Email Address and password. Ensure that you are trying to log on in Google Chrome, Safari, or Firefox.
  2. Click on the “Medical Clearances” tab on the left side of the screen.
  3. Under “Medical Clearances” click on the green “Update” button to upload a photo or scan of your Mandatory Health Form (PDF). Please ensure that all documents are legible.
    1. Please note:  If you are waiving your Meningococcal vaccine, please visit the bottom of the Medical Clearances page and submit your waiver under the Clearance Exemptions section. 
  4. Click on each the green “Update” button next to each type of immunization and enter in the dates of immunization. You must enter in all of your dates for the required immunizations in order for us to verify your record.
  5. Once you have updated all of your immunization dates, a staff member at SHC will review your record.

In order to add new documentation to update your immunization record please follow these instructions:

  1. Click on the "Medical Clearances" tab.
  2. Click on the green update button next to "Immunization Record". This will allow you to update your documentation only.
  3. DO NOT click on the specific vaccination you are trying to update.
  4. Please note you cannot change previously entered information, updating your document will prompt us to make the necessary changes to the appropriate vaccination(s).

Additional Immunization Information

Required Immunizations
  • One Tetanus Diphtheria booster or Tdap vaccine given within the last 10 years.
  • Two vaccinations against Measles, Mumps, and Rubella (MMR) given after age one and a minimum of 30 days apart, or provide laboratory proof of immunity.
  • Two vaccinations against Varicella (Chicken Pox) given after age one and a minimum of 30 days apart, or provide history of Varicella (Chicken Pox) disease, or provide laboratory proof of immunity.
  • Three vaccinations against Hepatitis B: dose two given 30 days after dose one, dose three given five months after dose two, or according to the Hepatitis accelerated schedule, or provide laboratory proof of immunity.
  • One Meningococcal vaccine A,C,Y,W-135, MenACWY (Menveo, MenQuadfi, or Menactra) given on or after their 16th birthday, or a signed Waiver Request Form (PDF).
  • Students under the age of 18 only: 4 doses of IPV (inactivated polio virus).
  • All incoming students are required to complete a TB Questionnaire through the student portal.
    • If you answer ‘yes’ to any of the questions on the TB Questionnaire, you will be required to complete a blood test to assess for exposure to tuberculosis. (we prefer the QuantiFERON Gold TB test)

    • We will accept QuantiFERON or T Spot testing from any lab within the United States, and the date of the laboratory test should not be dated before 7/1/23 for Fall 2024 entrants.  We will accept blood testing performed outside of the U.S. if the laboratory report documents the laboratory name and contact information, the student’s name and date of birth, the date of test and date of result clearly and accurately in English.  At our discretion, we may require repeat testing in the U.S.

Students who will not have turned 18 by 7/1/2024 for Fall 2024 entrants will be required to submit the following:

  • 4 doses of IPV (inactivated polio virus)
    • Dose 1: 6-8 weeks (no sooner than 6 weeks of age)
    • Dose 2: 4 months (at least 4 weeks after dose 1)
    • Dose 3: 6-18 months (at least 4 weeks after dose 2)
    • Dose 4: 4-6 years (at least 6 months after dose 3)

Medical or Religious Exemptions

Students seeking exemption from any of the immunization requirements for medical or religious reasons must submit:

Medical Exemption:

Medical exemptions must be accompanied by a letter from a medical provider supporting the exemption. Must include copies of MMR and Varicella Antibody Titers

Religious Exemption:

Religious exemption is considered if the responsible person objects in good faith and in writing that the immunizations violate their religious beliefs. Must include copies of MMR and Varicella Antibody Titers.

  • Upload your documents to the student portal under Downloadable Forms
  • Click on the green Upload button next to Immunization Record
  • Upload all supporting documents and exemption letters
  • Upload Titer Antibody Lab Results for MMR and Varicella
Registration Holds

Your student account will incur a registration hold if complete immunization records are not received by our office by the deadline required. After we receive your complete immunization records, the hold will be lifted. To see if your hold has been lifted:

  1. Login to the GWeb Info System.
  2. Select: Student Records & Registration -> Student Records Information Menu -> View Administrative Holds.
  3. If you see "Incomplete Immunization Status," it means that your record was either not received or is incomplete. If there is no Student Health hold listed, your record is received and complete.
